Assessment tools and Guidelines developed by the Regional Palliative Care Program and references for assessment tools used by but not developed by the program.

HOSPICE PALLIATIVE CARE UNIT ADMISSION  GUIDELINES: >
To outline the clinical criteria, process and information needed to refer persons for Hospice Palliative Care Unit admission. For the remainder of this document, the term “Hospice” may be used to describe a Hospice Palliative Care Unit.

Evidence-Based Care Management Tools And Guidelines: >
The purpose of evidence-based Care management tools is to assist clinicians in applying research evidence to clinical decisions, and to promote common practice for palliative care throughout the region.

Pallium Palliative Pocketbook: >
The Pallium Palliative Pocketbook was released in September 2008. It is a practical, peer-reviewed pocketbook intended to support safe, ethical, effective and accountable palliative clinical service interventions in various community-based settings of care (e.g., home, hospice, community hospital, long term/continuing care, community cancer care, etc.)

Obtaining Methadone Exemption License:>
The content of this package is intended to facilitate obtaining a general or patient specific Methadone Exemption License.The package includes: Methadone Exemption License application (to be faxed to Health Canada), an article by one of our palliative care consult physicians ( a discussion of clinical management of patients receiving methadone), Review of Methadone drug-drug interactions (Inhibitors and inducers of Methadone metabolism) , and a list of several Edmonton pharmacies that stock methadone.

If you are in the process of obtaining a Patient Specific Methadone Exemption License for a patient that is managed in collaboration with Edmonton palliative care program, a letter of support for your application can be provided to the College of Physicians and Surgeons of Alberta by the palliative care consultant involved in the same patient care.

Organ and Tissue Donation: >
A resource tool for clinicians to aid in having the conversation, frequently asked questions, criteria for donation and contact information. Also available is a printable brochure for your patients and families who may wish to consider sharing the gift of life with others as part of end of life care.
